If you are completing the registration form with the above link, please also complete our Data Sharing Form and Shared Administration form.
Please be aware that these two forms need to be completed prior to registration.

If you are unable to complete the registration process online, registration forms are available to collect from reception or you can download the forms below. You can complete and email the forms to us at boxsurgery@nhs.net or hand into reception.
Please complete the New Patient Registration Form, the Box Surgery TPP SystmOne Data Sharing and the Shared Administration Form.
You can hand the completed forms into Reception, post them to us or send via email.

You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

Organ & Blood Donation
After 30 September 2021, your GP surgery can no longer record your organ and blood donation preference. If you wish to record your preferences, you can still do so directly through the blood and organ donation online registration websites, or by phone:
